All new computers & management server showing "not monitored" SCOM 2012

We had a SCOM 2007 (not R2) server in place and running fine. All was well until it suffered a "critical hardware failure". Long story short the server was out of warranty and we couldn't find a replacement motherboard.

We set up a new server with SCOM 2012 SP1 and gave it the same name as the old SCOM 2007 server. After getting everything configured by following Microsoft documentation as closely as possible we decided to try to deploy an agent. We picked a server that had been monitored in SCOM 2007, uninstalled the SCOM 2007 agent, rebooted the server, then pushed out the agent from the SCOM 2012 server. It shows up fine in the monitor however the status is "Not Monitored". The Management Server is also showing up as not monitored. We tried the same action on another server with the same result.

We have tried clearing the health service cache on the agent servers and management server. The event log is clean on all servers. All the servers have been rebooted multiple times with no improvement. We've tried running the OM Cleanup Tool from the SCOM 2007 resource kit on the agent servers. All the proper management packs have been installed.

Is there some additional cleanup we need to do since we were not able to "gracefully" upgrade from SCOM 2007 to 2012?
